Optimization of Stent Deployment by Intravascular Ultrasound

چکیده
Intravascular ultrasound (IVUS) is a useful diagnostic method that provides valuable information in addition to angiography regarding the coronary vessel lumen, dimensions, plaque burden, and characteristics. The major use of IVUS in coronary intervention is to guide interventional strategies and assess optimal stent deployment. Since the introduction of the drug-eluting stent (DES), concerns about restenosis have decreased. However, high-risk lesion subsets are being routinely treated with DESs, and the incidence of suboptimal results after stent deployment, such as stent underexpansion, incomplete stent apposition, edge dissection, geographic miss, and the risk of stent thrombosis, have correspondingly increased. Thus, optimization of stent deployment under IVUS guidance may be clinically important. In this review, we focus on the potential role of IVUS in stent optimization during percutaneous coronary intervention and its clinical benefits.

Current Use of Intravascular Ultrasound in Coronary Artery Disease
Recently, most coronary interventions rely on visual assessment of the coronary lesions using angiography with all known inherent limitations. Intravascular ultrasound (IVUS) allows for the evaluation of the coronary pathology to obtain information about both the extent and nature of atherosclerotic plaque; thus, planning the intervention strategy is based on objective data. Following the adven...

OBJECTIVES We sought to characterize the differences seen after low or high pressure coronary artery stent deployment as assessed by intravascular ultrasound. BACKGROUND Until 1992, the success of stent deployment was assessed by angiographic criteria only, but in 1993 the procedure was expanded to include postprocedural single-use intravascular ultrasound imaging. Ultrasound criteria for suc...

The concept of stenting—intravascular mechanical support by transluminally placed endoprostheses—was first proposed by Charles Dotter 30 years ago.1 As an intrinsic property of the metallic endoprostheses, early thrombotic occlusion of freshly deployed stents has been a concern since their introduction to human coronary circulation in 1986.2 The unacceptably high rates (up to 24%) of thrombotic...
متن کاملEfficacy of non-compliant balloon post-dilation in optimization of contemporary stents: A digital stent enhancement study
Background There is no evidence from randomized trials for the benefit of routine non-compliant balloon (NCB) post-dilation after stent deployment. Despite being the gold standard, intravascular ultrasound is infrequently performed due to time and cost constraints and a suitable alternative technology is required for routine assessment of stent expansion. The purpose of this study was to assess...
متن کاملPredictors and outcomes of stent thrombosis: an intravascular ultrasound registry.
AIMS To investigate whether intravascular ultrasound provides additional information regarding the prediction of stent thrombosis, a retrospective multicentre registry was designed to enrol patients with stent thrombosis following stent deployment under ultrasound guidance. METHODS AND RESULTS A total of 53 patients were enrolled (mean age 61+/-9 years) with stable angina (43%), unstable angi...
